**Concept Map: Collaborative and Advocacy Opportunities**

Design a graphic, or visual model (concept model) depicting advocacy strategies to support the policy process via a conceptual model or map of
local community response to protect population health. Actions of advocacy for health equity, health policy and collaborative opportunities and partnerships should be demonstrated. Demonstrate collaborative opportunities and partnerships for individuals and organizations that challenge barriers that impact population health outcomes and health equity. In order to do this, you should show other disciplines that may be involved, organizations and populations at risk related to the situation. Contexts for ideas and consideration are demonstrated in the domains below, but are not limited.

Examples of topics may be community emergencies and disasters such as: lead water crisis in a small community, excessive heat wave, county wide flood, electrical outage in a housing rent controlled development or, county schools influenza outbreak. You can be creative. You do not have to demonstrate all domains, but rather use the domains as ideas for your advocacy and action.

Further there should be sufficient uniqueness to relate to the topic versus simply using the domain titles alone in the model. Title must include topic and location/ county/ state. The product is a single PPT slide, or Diagram in a Word document, presented as a conceptual model. There should be 6–10 nodes of advocacy/ action presented with relational indication via shape or direction. A node is one of the items; circle/ square with a word in it. You may have sub-topics or bullets within or below each node; however, this is a high level overview assignment. No paragraphing or long sentences. How you design it, demonstrate concepts, and come up with actions is your choice.

You should search examples of concept maps and research the design and content. Pick your topic. Identify key concepts and incorporate the rubric. Organize with shapes, lines, or SmartArt and fine tune it. Create a main concept. Using a simple Smart Art design is sufficient but you are not limited to this and can make your own shapes to best demonstrate the context as you see it. Place the reference list without citation in the speaker notes in PPT, or below the diagram if using Word.

Use concepts and context from the Public Health Emergency Preparedness Program:

Population Health Proposal – Part 2

Community-Level Intervention to Address Adult Obesity

    Michelle Murray

    Herzing University

    Dec 5, 2025

    Definition of the Population

Adults aged 18–65 receiving outpatient mental health treatment.

High prevalence of depression, trauma, and emotional dysregulation.

Increased vulnerability to emotional eating and sedentary behaviors.

Limited access to nutrition education and community wellness resources.

Weight stigma contributes to long-term health risks (Roberts & Polfuss, 2022).

The target population comprises adults aged 18 to 65 years who are the recipients of outpatient mental health services on an ongoing basis, and this is a population that has a complicated psychological need that shapes lifestyle practices. Most of them have depression, anxiety, and trauma, which are some of the factors that lead to emotional rather than healthy coping mechanisms like emotional eating. Such people also tend to have little access to the correct nutrition information or community wellness resources, which leads to an impediment in making healthy choices. Stigma on weight only adds to their problems by decreasing incentives to perform physical activity or medical care. These aspects combine to put this population at the increased risk of obesity and other chronic health outcomes

    2

Overview of the Significant Health Problem

Obesity is highly prevalent among adults with mental health conditions.

Emotional eating, stress, and trauma increase obesity risk factors.

Weight stigma leads to avoidance of physical activity and healthcare.

Obesity contributes to chronic diseases such as diabetes and hypertension.

Early-life stigma predicts long-term obesity risk (Roberts & Polfuss, 2022).

The problem of obesity in adults is severe and is on the rise among patients under mental health care as people are highly susceptible to weight gain due to the presence of psychological factors. Unhealthy food habits and lack of physical activity are usually caused by emotional eating, traumatic background, and chronic stress. The stigma of weight also dishearten patients to seek support, engage in physical activities or take part in preventive healthcare. Diabetes, hypertension, and cardiovascular disease are also chronic illnesses that are closely linked with obesity, increasing the impact of health dangers in the long-term. The studies show that the impact of stigma in early life can influence health behaviors throughout life, and the problem of obesity in adults is a significant topic in mental health populations.

    3

    Range of Evidence-Based Interventions

Cognitive Behavioral Therapy shown to reduce emotional eating (Smith et al., 2022).

Motivational Interviewing improves readiness for lifestyle changes (Hayes & Forman, 2021).

Community nutrition education increases knowledge and healthy choices.

Group-based programs build peer support and promote sustained behavior change.

Policy-focused interventions can reduce structural barriers to healthy living

The literature presents a number of potential ways of combating obesity among adults with mental illness. CBT proves effective in minimizing emotional eating and promoting behavioral change, which makes it a highly acceptable treatment. Motivational Interviewing (MI) enhances change preparedness and helps to increase self-motivation. Health literacy and decision-making can be enhanced in community nutrition programs. Interventions in a group incorporate peer support, accountability, and a learning experience that is prolonged and generates and maintains progress. The multi-level approach to obesity is necessary because policy level interventions deal with structural barriers, including access to healthy food or safe environment to exercise.

    4

Selected Intervention (Yellow Wedge: Health Teaching)

Delivered through mental health clinics and community centers.

Focus on nutrition literacy, emotional eating, and stigma awareness.

Incorporates skill-building on meal planning and physical activity.

Uses group format to enhance engagement and accessibility.

Supported by evidence showing community education improves behaviors (Hayes & Forman, 2021).

The chosen intervention is a community-based program of nutrition and wellness workshops based on the category of health teaching offered by the Yellow wedge. These sessions will be offered in the mental health clinics and community centers and therefore, will be accessible to various participants. These workshops will be aimed at enhancing nutrition literacy, emotional eating, and making people more aware of the effects of weight stigma. Practical skill-based learning exercises, including meal planning and physical activity education, are used to put knowledge into practice. A group format enhances involvement, social support and creates a wider reach of the intervention. Community education is effective according to research on enhancing long-term health behavior.

    5

    Rationale for Intervention Selection

Addresses community-level contributors to obesity, not just individual factors.

Aligns with psychological needs of outpatient mental health participants.

Reinforces skills used in CBT and supports emotional regulation.

Increases reach by serving groups rather than individuals one-on-one.

Supported by research linking education with long-term behavior change (Smith et al., 2022).

This community-based intervention is the best since it considers the greater environmental and psychosocial factors that lead to obesity as opposed to considering individual behaviors. The outpatient mental health setting requires adults to be provided with structured and supportive learning environments that supplement emotional regulation skills that they acquire during therapy, and educational workshops are the most logical choice. There is also a possibility of reaching more, cost-effectiveness and peer support in group-based education. The intervention will complement psychological skills that are taught during CBT and help to make healthier choices in the non-clinical setting. The facts always indicate that community health education enhances motivation, promotes long-term behavioral change, and improves the overall wellness outcomes.

    6

    Implementation Plan: How & Where

Conduct biweekly sessions in clinic classrooms or community centers.

Collaborate with public health educators and mental health clinicians.

Use trauma-informed approaches to address stigma-related barriers.

Provide printed materials, meal guides, and local food resource lists.

Incorporate participant feedback for continuous program improvement

It will be implemented in outpatient mental health clinics, community rooms, and partnering facilities in the public health in order to make it accessible. The group sessions will be conducted twice a month at a time that is predictable and convenient to the group. Cooperation between the mental health clinician and the public health educator will make sure that the curriculum incorporates the psychological and behavioral aspects. Trauma-informed principles will be used to facilitate conversations to decrease discomfort caused by stigmatization and facilitate inclusivity. The participants will be provided with printed nutritionists, lists of local food resources and behavior-tracking tools. Feedback of the participants will be used in the continuous improvement so that the program is culturally acceptable, interesting and flexible to address the needs and preferences of the community.

    7

    Anticipated/Desired Effects

Increased nutrition literacy and awareness of healthy eating.

Reduced emotional eating and improved coping behaviors.

Decreased internalized stigma related to weight and body image.

Higher engagement with preventive health and wellness resources.

Long-term reductions in obesity risk factors supported by research (Katterman et al., 2020).

The intervention is designed to enhance the health outcomes of the community as it equips the participants with the knowledge and skills required in order to lead a healthier lifestyle in terms of eating. Greater nutrition literacy will enable the adult population to make better food decisions, whereas better coping mechanisms will decrease emotional eating. Addressing weight stigma will be predicted to increase self-esteem and motivate the use of healthcare and wellness services more. The intervention encourages increased engagement in health promoting behaviors by encouraging positive group processes and realistic health education. These changes will, in the long term, reduce the risk factors of obesity, enhance psychological health, and positively affect the quality of life.

I need someone to respond to this discussion post following the instructions and rubric! No plagiarism or AI use!
    Discussion post: Initial Response
    Nguyen et al. (2020)
    **Study Description**
Nguyen et al., (2020), conducted a population-based case-control study based on patients that were part of the Epidemiology Strengthened by histoPathology Reports in Sweden (ESPRESSO), the Swedish Patient Register, and Prescribed Drug Register to identify a correlation between patients diagnosed with irritable bowel disease (IBD) and the use of antibiotics a year prior to the patient being diagnosed with IBD. The researchers also included the IBD subtypes of ulcerative colitis and Crohn’s disease for this research. With the use of ESPRESSO, the researchers found nearly 24,000 IBD cases through the years 2007 to 2016, in which these cases had matched controls and unaffected siblings for comparisons. The researchers utilized logistic regression to estimate multivariable-adjusted odds ratios (aORs) and 95% confidence intervals (CIs).
    **Strengths and Limitations**
Nguyen et al.'s population-based case-control study had a primary strength in that it was a large, nationwide sample drawn from the Swedish population registries that looked at nearly 24,000 cases of IBD cases that had additional robust matching controls and full siblings. Such a large sample allows for the reduction in selection bias and accounts for shared genetic factors and environmental factors that could have contributed to the diagnosis of IBD. While the study had a strength in numbers, there were some limitations to the study that needed to be addressed. The primary limitation that has been identified is that the study’s observational design limits its ability to establish causation, and there is then a potential for residual confounding, such as differences in infection severity or unmeasured lifestyle factors. Not knowing the significant medical background of each patient does not allow for a full scope analysis of the causation of IBD, which could be related to socioeconomic factors and behavioral factors that can increase the prevalence of IBD. Additionally, the prevalence of IBD in Sweden is found to be elevated with lower antibiotic dispensation patterns when compared to other countries, which can be associated with another limitation of the research.
**Population, Data Sources, and Epidemiological Measures**
The population that was utilized for this research included individuals who were 16 years of age or older who lived in Sweden. The patients had a compatible IBD diagnosis, including Crohn’s disease and ulcerative colitis. To allow for appropriate comparison, the researchers used two sets of controls: matched population-controls that were drawn from the general population and full siblings of the IBD cases. The authors of this study utilized the Swedish registries, including pathology/histology reports, which was from the ESPRESSO, to identify valid IBD diagnoses. The authors then identified antibiotic exposure from prescription/dispensation records that were recorded through the systemic antibiotic dispensaries. The epidemiological measures incorporated into the study included the odds ratio, which compared the odds of prior antibiotic use among IBD cases versus the controls, and they also utilized a dose-response relationship to measure the cumulative antibiotic exposure. Additionally, the authors incorporated a sensitivity analysis that looked at a two-year lead-in period, which was to ignore the antibiotic dispensation in two years before the IBD diagnosis, and the researchers found that this had similar risk estimates compared to the one-year dispensation before diagnosis.
    **Design Appropriateness**
The study design utilized by Nguyen and associates (2020) was appropriate. The case-control study design was well-suited for examining exposure and association, in which the researchers efficiently assessed antibiotic exposure using existing registry data and diagnoses of IBD.
    **Researchers’ Conclusion**
I would likely not support Nguyen and associates’ (2020) conclusion that antibiotics may cause IBD. I believe other factors would need to be understood before having high confidence in the prevalence of antibiotics causing IBD. An example of other factors would be socioeconomic factors, which were highlighted in the limitations of the study. Additionally, the duration during which the patients were on antibiotics was not established during the study, which should also be taken into consideration, given that some patients may be on antibiotics for one week versus one month, depending on the case.
    Palència et al. (2020)
    **Study Description**
Palencia et al. (2020) produced an ecological study of trends to identify the effects of inequalities that have been aggravated by economic recession and the mortality rates within certain diseases, with their primary focus on lung, colon, prostate, and breast cancers within nine European urban areas. The study looked at three periods, 2000-2003, 2004-2008, and 2009-2014, and observed the relationship between socio-economic deprivation and cancer mortality. The study emphasizes the effects of socioeconomic inequalities and how these inequalities shape patterns of cancer mortality across urban neighborhoods, revealing that residents of more deprived areas consistently experienced higher mortality for several cancer types and that these disparities persisted across all three time periods examined.
    **Strengths and Limitations**
A key strength of this study is that it was quite large and included a multi-city design throughout nine European urban areas, while using small-area deprivation indices and Bayesian spatial models over multiple time periods that robustly examined the socioeconomic inequalities in cancer mortality before and after the 2008 economic crisis. The researchers also incorporated some of the major cancer types, analyzing them by patient sex, and comparing these patterns across urban contexts. A limitation to this study would be that the post-crisis data only included six years after the initial crisis and did not take into account longer-term effects from the crisis on cancer inequalities. Due to the latency periods that occur with the identified cancers within this study, additional timeframes could have been added to the study.
**Population, Data Sources, and Epidemiological Measures**
For this study, the researchers utilized residents from Athens, Barcelona, Berlin-Brandenburg, Brussels, Lisbon, London, Prague, Stockholm, and Turin during three separate time frames of 2000-2003, 2004-2008, and 2009-2014. Palencia and associates (2020) focused on adult participants and analyzed adult men and women separately. The data sources for this study included census records, mortality registers, socioeconomic indicators based on the census data, and the use of a composite deprivation index that was created through principal components analysis. The study utilized epidemiological measures to include standardized mortality ratios, ecological regression models, and relative risks.
    **Design Appropriateness**
The ecological trend design that was utilized for this study was appropriate for looking at how socioeconomic deprivations are correlated to cancer mortality.  The study examined population-level patterns, geographic inequalities, and changes that occurred following the 2008 economic crisis, in which the use of the ecological design was appropriate since it allows for a better understanding of the structural and contextual factors that cannot be properly assessed from an individual level.
    **Researchers’ Conclusion**
While the researchers concluded that their hypothesis was not validated by the study, they concluded that there needs to be more emphasis on the socioeconomic measures and the correlation with mortality from cancer, which I agree with. The researchers emphasized that, given the information that was derived from their research, more local health programs that are targeted, equity-focused, and responsive to area-level socioeconomic disparities are needed to help reduce cancer mortality inequalities across urban communities.
