Please approach this assignment as an opportunity to integrate instructor feedback from part I and expand on ideas adhering to the components of the MAP-IT strategy. Include necessary levels of detail you feel appropriate to assure stakeholder buy-in.

    **Directions**

You have already completed the steps 1-4.
**Do not resubmit part 1.** Make sure you incorporate any suggested revisions or improvements into your submission according to your instructor’s comments.

**To assist in maintaining harmony between Part I and 2 here you have a reminder of the previous paper outline:**

    · 3. Provide a review of literature from scholarly journals on evidence-based interventions that address the problem. After completing a library search related to effective interventions for your chosen health promotion activity, you will write a review that evaluates the strengths and weaknesses of all the sources you have found. You might consult research texts for information on how to write a review of the literature found in your search.

    · 4. Select an appropriate health promotion/disease prevention theoretical framework or conceptual model that would best serve as the framework guiding the proposal. Provide a rationale for your selection which includes a discussion of the concepts of the selected model

**For this assignment develop criteria 5-8 as detailed below:**

**You will submit just this section 5-8 as an essay. Please do not resubmit Part 1.**

Use a presentation page. Start the body of content with topic 5.

    · 5. Propose a health promotion program using an evidence-based intervention found in your literature search to address the problem in the selected population/setting. Include a thorough discussion of the specifics of this intervention which includes resources necessary, those involved, and feasibility for a nurse in an advanced role.

    · Be certain to include a timeline. (3 to 4 paragraphs- you may use bullets if appropriate).

    · 6. Thoroughly describe the intended outcomes. Describe the outcomes in detail concurrent with the SMART goal approach.
**The SMART goal statement should be no more than one sentence** (1 paragraph).

    · 7. Provide a detailed plan for the evaluation of each outcome. (2-3 paragraphs).

    · 8. Thoroughly describe possible barriers/challenges to implementing the proposed project as well as strategies to address these barriers/challenges. (2+ paragraphs).

    **Health Issue: High Blood Pressure**

Hypertension is a severe global public health concern, especially in low- and middle-income nations, and has a profound impact on cardiovascular outcomes (Goorani et al., 2024). Hypertension affects nearly 47.7 percent of adults in the United States, and differences in the incidence and management of this condition can be observed across racial, ethnic, and socioeconomic groups (Fryar et al., 2024). As an example, hypertension rates are higher in non-Hispanic Black people (48.8) than in non-Hispanic White people (37.6) and Hispanics (27.9) (Sells et al., 2023). High blood pressure is a significant predisposing factor to heart failure, stroke, and kidney failure. Although these are very alarming statistics, a significant number of people are unaware of their condition, and control rates are low. The proposal seeks to control hypertension that has not been managed through community-based screening, self-monitoring and taking medicine, and connection with primary care. The targeted, quantitative change is a 15% relative improvement in the percentage of enrolled individuals who control their blood pressure (<130/80 mm Hg) in six months, confirmed through clinic-validated home measurements and chart recordings.

**Vulnerable Population: Hypertension in Underserved Communities**

The target population of the proposed health promotion program is the vulnerable population that consists of low-income and minority groups, especially non-Hispanic Blacks and Hispanics, who have a disproportionately high hypertension burden. Such populations are at risk of poor nutrition, lack of access to healthcare, stress, and education on the risks of hypertension. Studies have revealed that these groups have increased hypertension rates and less chance of appropriate care or treatment (Sells et al., 2023). Social determinants of health such as poverty, insurance and low levels of education, pose a high risk of developing hypertension and the poor control of hypertension. Also, the complications of hypertension management are exacerbated by the stigma of chronic illnesses among particular populations. Meticulous health interventions focusing on these aspects will be important in curbing the prevalence of hypertension among these susceptible groups and the availability of preventive health services and treatment to everyone.

    **Literature Review**

Persell et al. (2020) detail one rigorous intervention in their randomized clinical trial comparing a smartphone coaching app and home blood pressure monitor to a tracking app and monitor in adults with uncontrolled hypertension. After six months, the systolic blood pressure of the coaching-app group decreased on average by 8.3 mm Hg (compared to the baseline) and the tracking-app group dropped on average by 6.8 mm Hg, but the adjusted between-group difference was not statistically significant (Persell et al., 2021). Notably, individuals who adopted the coaching app had increased self-efficacy in the management of their blood pressure. The strengths of the study are its randomized design, use of mobile technology and home monitoring in over six months, and measurement of physiological and behavioral outcomes. The limitations, however, include the small effect size and the likelihood that the sample size was not sufficiently large to identify smaller, but clinically significant differences.

Likewise, an investigative randomized controlled trial by Beger et al. (2025) evaluated the impact of the “Manoa’ digital blood pressure coach app in the primary care environment (non-hospital). The app was used with patients in the intervention arm, and it integrates guideline-based home BP with personalized coaching on lifestyle changes, whereas control patients were provided with standard care. The intervention group reported more office systolic blood pressure reduction than the control group over a period of 90–150 days, and it can be concluded that digital coaching plus structured home monitoring can make a significant contribution to the management of hypertension in clinical practice. The authors concluded that the app was more effective than standard care when it comes to BP control. One of its strengths is the real-world design, as it indicates how such an intervention could be used in ordinary clinical practice. Nevertheless, the comparatively brief follow-up hinders inferences regarding long-term effectiveness and sustained behavior change – and, as with most app-based interventions, there could be digital access or usability obstacles that do not allow all patients to get the full benefit.

    **Evaluation of Evidence**

All these works present a strong argument in favor of the potential of digital health interventions to manage hypertension, especially when home blood pressure monitoring is used alongside behavioral coaching. According to the Persell et al. trial, a coaching application can enhance patient self-confidence in the management of their own blood pressure, although the reduction in measured blood pressure may have been modest. The pragmatic trial provides real-world validation to prove the fact that such an app can be applied to primary care, and it can be integrated into guideline-based monitoring. Mobile interventions have several advantages such as scalability, low marginal cost, and extensive reach.

Conversely, there exist obvious limitations. The effect sizes are low which casts doubts on clinical significance. The long-term sustainability is not easy to evaluate due to the relatively short periods. Furthermore, digital interventions are already biased towards patients who have access to smartphones and are digitally literate, which may exclude other subpopulations that may be the most vulnerable. Lastly, results can be related to self-reported behavior, which may be biased.

**Health Promotion Model: Health Belief Model (HBM)**

The Health Belief Model (HBM) is chosen as the theoretical framework due to its constructs, perceived susceptibility, perceived severity, perceived benefits, perceived barriers, cues to action, and self-efficacy, which are aligned with the hypertension prevention and control activities (Alyafei & Carr, 2024). A large number of hypertensive adults are asymptomatic and thus they underestimate the risk of the disease and its severity in case of uncontrolled illness; this risk will be made salient and concrete through HBM-based education as it will reveal risk trends based on home monitoring and clarify the tangible benefits of control, such as decreased risks of stroke and heart diseases. The program will contain cues to action that will be automated alerts, community screening activities, and community health worker outreach, whereas interventions to reduce perceived barriers will focus on cost, transportation and health literacy.

The HBM focus on self-efficacy also contributes to intervention elements in which practical skills are developed: training on home BP measurement that is proven, medication management, diet and activity goal-setting, and peer-support sessions to enhance confidence. The program has a combination of individual-level HBM tactics and structural enablers, such as providing home monitors, community health worker navigation, telehealth escalation pathways and connection to low-cost medication support, which meets the behavioral determinants and social constraints. This theoretically based, multilevel design is aligned with the best evidence available and is likely to enhance engagement and measurable blood pressure control in the vulnerable population targeted. The program will focus on enhancing hypertension control by 15 percent among program participants in six months. The intention-to-treat analysis and blood pressure results that are proven by clinics will also be used to evaluate the results rigorously.

    **Health Issue: High Blood Pressure**

Health Promotion & Role Development in Adv. Nursing Practice

    November 18, 2025

Lastly, results can be related to self-reported behavior, which may be biased.

**Health Promotion Model: Health Belief Model (HBM)**

    **## DecisionMakingProcessPaperDue.docx**

    **Decision-Making Process Paper**

    Instructions

In this assignment, you will be writing a 6–8-page paper identifying [
**maximum page limit is 8 pages- do not exceed as it will incur a penalty**], comparing and contrasting at least 3 different decision-making approaches of care from a multidisciplinary perspective using a pertinent and significant topic in nursing [The following topics are NOT allowed and will not be approved:  shortage and nurse turnover, nurse staffing ratios, unit closures, and restructuring, use of contract employees (i.e., registry and travel nurse)]. The topic
**MUST BE APPROVED** **by your nursing instructor before writing the paper. Your topic was approved in week 3- please refer to week 3's approved topic.** You are
required to use APA 7th edition format, and the page count, appropriate headings, the title page, and the reference page. Reflect on your decision-making practices and the implications this will have regarding differing approaches taken by nursing leaders and staff nurses/clinical practitioners in decision-making regarding issues in practice.  To complete this assignment, do the following:

    1. Select and describe an issue in nursing that impacts nurses at all levels from bedside to advanced practice. Make sure to provide an adequate background and its significance to all levels of nursing using a strong review of the literature. Additionally, this should include a thorough literature review that provides a good understanding of what the cause of this problem is; what is suggested to address this problem. Essentially, this review must support your choices of the decision-making models and this needs to include how all levels of nursing practice (the bedside nurse to the APN are impacted by the problem).

    2. Identify how you would expect a nurse leader/manager, a bedside/staff nurse and an Advanced practice nurse (CNO or NP) to address your selected issue using at least 2 [3 ideal] different decision-making approaches which are clearly supported by the literature review. This must first include aa detailed analysis/overview of the selected decision making models and how they are used in practice. Then a detailed analysis of how these decision-making approaches/models would address the problem/along with a detailed analysis of how each of the three selected models would be applied to address the selected issue/problem making sure to incorporate this at all levels of practice. One of the decision-making models may be more applicable to the bedside nurse; whereas the other more applicable to the APN; however, all levels of nursing practice need to be addressed with at least one of the chosen 3 decision-making models. However, this does not mean that each decision-making model will apply to all levels of practice only that in this discussion needs to include how each level of nursing practice would utilize the decision-making model to address the problem. For instance, one decision making model may apply to the bedside nurse; the second decision making model may be more applicable at the level of APN (CNO, NP, etc) and the other decision-making model may be best applied to the nurse manager. This section should be detailed; meaning specific, well thought out, realistic solutions of how the nurse (at the level identified; this may include more than one level for the same decision-making model or not) would address the problem using the decision-making model is provided and well supported by the literature. of the benefits and limitations of various decision-making approaches that are applied appropriately to the level of nursing practice to address the selected issue/problem. This is informed from course readings, literature, clinical scenarios and other evidence-based and scholarly sources.

    3. Prioritize the best decision-making approach/model to address the selected problem that is grounded in best practice. This should provide a well-supported rationale for this choice that includes a good comparison and contrasting of the 3 chosen decision-making models that clearly illustrates why the choice was made, that is consistent with the decision making model and how it would address the chosen topic or problem.

    4. Identify a possible funding source that would addresses your approach to solving the problem. For instance, if the best approach to solving the nurse-to-patient ratio problem is through the use of SDM and shared governance, perhaps you may seek a local funding source to provide you with funding to implement a small EBP or research project that would be funded by a local nursing organization such as Sigma Theta Tau or perhaps if it was geared more towards assessing the value of this model towards NP in practice than seeking a local organization that supports funding for projects being implemented by practicing NP? If seeking money for research this would need to be overseen by a nurse with a DNP or a PhD or someone with the credentials to do so that are part of their scope of practice. Consider looking at federal, state, or local organizations. For example: There are many grants available through the CDC,HRSA, etc. Again, this need to be specifically applied with sufficient details

    5. Use at least
**10 current scholarly references** [not older than 5 years] other than your main text. Uses APA 7th edition formatting.

The Use of Yoga to Manage Symptoms in Patients with Multiple Sclerosis

    I. Introduction

    A. Background Information: Multiple sclerosis (MS) is a chronic autoimmune

disease affecting the central nervous system, leading to symptoms such as fatigue,

pain, spasticity, balance issues, and depression (Graves et al., 2023). Complementary

therapies, such as yoga, are increasingly being explored in nursing practice to improve

    quality of life.

    B. Statement of the Problem: Traditional pharmacological interventions often

inadequately address non-motor symptoms and side effects, prompting the need for

    integrative, nurse-supported interventions like yoga.

    C. Thesis Statement: Yoga, as a mind-body intervention, offers a safe, accessible,

and evidence-based approach for bedside nurses and advanced nurses to

collaboratively manage MS symptoms, enhance patient autonomy, and inform clinical

decision-making through biopsychosocial and self-efficacy frameworks (Naisby et al.,

    2023).

    II. Literature Review/Background

    A. Overview of Existing Research: Review of randomized controlled trials and

systematic reviews (Lysogorskaia et al., 2023) demonstrating yoga’s effects on

fatigue, mobility, mood, and pain in MS populations.

    B. Key Theories and Models

    3

    1. Biopsychosocial Model (Engel, 1977): Frames symptom management as an

interplay of biological, psychological, and social factors; yoga addresses all

    three domains (Lugg, 2022).

    2. Self-Efficacy Theory (Bandura, 1997): Yoga practice enhances patients’

confidence in managing symptoms, supporting long-term adherence and self-

    care (Su, 2024).

    III. Main Body/Discussion

    A. Section One Clinical Context and Symptom Burden in MS

    1. Prevalence and impact of fatigue, spasticity, and depression in MS.

    2. Limitations of pharmacological management and role of non-pharmacological

    interventions.

    B. Section Two Comparative Analysis of Nursing Roles in Yoga Integration

    1. Bedside Nurse Role: Patient education, safety screening, gentle pose

modifications, and monitoring during inpatient or outpatient sessions.

    2. Advanced Practice Nurse/Nurse Leader Role: Protocol development,

interprofessional collaboration (e.g., with physical therapists), outcome tracking,

and policy advocacy for yoga programs.

    C. Section Three Application and Implications

    1. Case example: 6-week adapted yoga program in an outpatient neurology clinic.

    2. Benefits: Improved fatigue scores (MFIS), balance (Berg Balance Scale), and

    mood (PHQ-9).

    4

    3. Limitations: Accessibility, contraindications (e.g., severe spasticity), and need for

    trained instructors.

    4. Recommendations: Nurse-led training modules, tele-yoga options, and integration

    into care plans.

    IV. Conclusion

    A. Summary of Key Points: Yoga serves as a multidimensional intervention aligned

with biopsychosocial and self-efficacy principles, supported by nursing leadership at

    all levels.

    B. Implications and Next Steps: The Final paper will propose a nurse-driven yoga

protocol with measurable outcomes, advocating for institutional adoption and further

    research on long-term adherence.
